I accepted all the ladder quests and I'm sure did not forget a single one from Level 10 to 60. We passed floor 49 and died at floor 50. But when I tried to claim my rewards, was short of one pack. I can't claim my lvl 35 pack...

common mistake, you weren't in floor 35 when your team members killed the enemies there, either you're lagging or just late, I know that some people that really like to chat during ladder and slowly getting to next floor, they won't get the kills.
