About "Planes"

What is the exact order of the plane?
I know mortal is the first one but what is after that and so forth?
I tried looking in the guilds but couldn't find them.

Also, would you know if everything disappears once you are reborn?
I heard that once you are reborn you lose your pet and weapons and everything.

You lose nothing, you just go back to level 1, with extra attribute and skill points to start you off with.
