Last post stated mentors cannot change names. When I made my name R2Tormentor before who changed my name then? R2Mentor led me to believe it was him and he was the only one that addressed me about it and it was changed.

Mentors and mods can't change names. They have very little power, but they do have a direct line to the people who can do things like that. So if one of the mentors reported your name, he could certainly claim credit for it, if only indirectly.
