It isn't a 100% drop, but it will drop in Sacrificial Relics, Killer's Den and Psychodelica.
Nightmare difficulty has a higher drop rate than Hard. In Normal difficulty it doesn't drop.Helpful Links

you kidding me i started to beleive u removed the drops from any dungeon i have been doing all dungeon on nighmare mode for a week and havent got any dc and i need only1(even used the drop increase title still nothing, untill last night i saw some1 say that they got one from sr again)Comment
