Dragon sword frag won't spawn

The Dragon sword frag won't spawn so i can't continue the main quest the only other solution i can think of is to make a new character and i might just have to do this

Do you mind clarifying this issue? Or providing screenshots of the quest so we can figure out the issue? It's the first I'm hearing of this particular problem.
