If she get suspectious of anything she gonna get more sneaky with her tactics i rather catch her in the action of doing something. But i do have 1 question. she was banned a few times i believe (i know once) but she keeps changing her IP address is there a way to ban her for good?

Changing your IP address does not bypass a ban. If said player was previously banned, it was only temporary and she returned after her ban had expired.

Her main got banned. what she is doing is making all these level 1s to get back on and threaten to kill me and a few others.

You can choose to report said alts for harassment via ticket (http://support.r2games.com/), but your best bet would be to just ignore them.
