glitch

hi, someone killed me just as i was going through a portal/teleport thing and it completely glitched out. i wasn't offered the chance to revive, i hadn't dropped anything, i couldn't use potions, i just sat there???? does anyone know about this, and is it my computer or is it a game glitch
