dragon totem gives rage.
Totems (whatever it is) gives you additional stats when they are active, when they vanish, the additional stats they give also vanishes.
Ofc, the player with higher agi will attack first.

and dont forget about all the godly gear skills, if someone is wearing the boots and their agility gets boosted up, then they may hit first when normally they dont.
