Click that, skim through 1st-3rd images. (you can click on them to enlarge)

picture 1: click on open new ticket
picture 2: click on category of what the issue you're reporting is
picture 3: fill out forms.

For attachments; it just brings up the normal attachment thing; but I recommend putting the screenshots into imageshack or some other site because the attachments have size limits.

It will not.
They are implementing those manually based off information received from Joyopark players, if they successfully responded to the needed information their characters will be restored during maintenance on Thursday at 2 AM PST.
